For tests like EtG and/or beyond a 10-panel screen, 150mg is suggested. Using a jeweler’s scale to weigh the sample is advised. If scalp hair isn't available, a similar volume of body hair can be collected. 'Head hair' refers specifically to scalp hair, whereas 'body hair' encompasses all other hair variants (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test encompasses four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the first steps of inputting a sample into the lab's system. This step ensures that the sample was correctly sealed and transported, assigns a random LAN (Laboratory Accessioning Number) to it, and fulfills any data entry requirements absent from an electronic chain of custody system.

Screening conducts a preliminary assessment for illicit substance presence. Though cost-efficient for discarding potential negative drug use amongst most samples, a positive screen mandates further confirmation for legal acceptance. Samples that show a presumptive positive in this stage need secondary verification.

For a sample with a presumptive positive at Screening, additional hair is taken from the original sample for Extraction. This stage draws drugs from hair at a considerably lower concentration compared to alternate methods (e.g., urine or oral fluids), explaining the complexity of hair drug screening procedures.

Any affirmative result from screening undergoes Confirmation through GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, presumptive positive samples are cleansed as necessary. The entire procedural flow, from Accessioning through Confirmation, is reviewed under CAP (College of American Pathologists) Hair criteria and under ISO / IEC 17025 standards of accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
