By collecting a 1.5-inch section of roughly 200 hair strands (approximately the width of a #2 pencil) nearest to the scalp, it yields 100mg of hair, which is the optimal quantity for conducting screenings and confirmations. For EtG, additional tests, and/or those exceeding 10 panels, a sample of 150mg is advised. It's suggested to measure the hair using a jeweler’s scale. In the absence of scalp hair, a comparable quantity of body hair can be used. Note that when scalp hair is mentioned, it exclusively refers to hair from the head. Body hair encompasses all other hair types (including facial, axillary, etc.).

Process Overview

The lab processing of a drug test result involves four main stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the first step which involves the integration of a sample into a lab system. This phase ensures that the sample was securely sealed and transported, assigns a random LAN (Laboratory Accessioning Number), and completes any additional data entries that are not included in the electronic chain of custody system.

Screening is an initial rapid assessment for detecting drugs of abuse. Although Screening is an economical means to negate drug use in most samples, a positive finding must be confirmed for judicial acceptability. Samples with presumptive positive results in Screening mandate further confirmation.

Should a sample yield a presumptive positive during Screening, additional hair is retrieved from the original sample for Extraction. At this phase, drugs are isolated from hair at a significantly lower concentration compared to other methods (for instance, urine or oral fluid), highlighting hair drug screening as the most challenging method to execute.

Confirmation of any positive Screening outcome is carried out using GC/MS, GC/MS/MS or LC/MS/MS. All samples showing presumptive positive are cleansed prior to confirmation as required. The entire laboratory process, from Accessioning to Confirmation, is meticulously reviewed according to both the CAP (College of American Pathologists) Hair criteria and the ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
