If scalp hair is not present, a comparable quantity of body hair can be used. "Head hair" pertains solely to scalp hair, whereas "body hair" encompasses all other hair varieties (facial, axillary, etc.).

Process Overview

The four key stages in the lab processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial incorporation of a sample into a lab's system. This step ensures the sample was appropriately sealed and dispatched, assigns a random LAN (Laboratory Accessioning Number), and finalizes any pending data entry absent from an electronic chain of custody system.

Screening serves as a preliminary assessment for substances of abuse. Although Screening is an economical technique to negate drug use in the majority of cases, a positive finding necessitates confirmation for legal acceptance. Any specimens displaying presumptive positivity in Screening need further confirmation.

Should a sample manifest as presumptive positive in the Screening phase, additional hair is extracted from the original sample and poised for Extraction. In this phase, drugs are drawn from hair at a significantly reduced concentration compared to other methodologies (e.g., urine or oral fluid), underscoring hair drug testing as a particularly challenging process.

Confirmation involves using GC/MS, GC/MS/MS, or LC/MS/MS for verifying any positive screening result. All presumptive positive samples receive a wash, if necessary, prior to confirmation. The complete laboratory procedure from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.
