When sampled near the scalp, hair can reveal drug and alcohol usage details for nearly 3 months. It is straightforward to collect, somewhat tamper-resistant, and convenient to ship.

By collecting a 1.5-inch segment consisting of around 200 strands of hair (akin to the thickness of a #2 pencil) nearest to the scalp, you accrue 100mg of hair, which is perfect for testing and validation. To test for EtG, additional substances, and/or conduct extended panel tests (beyond 10), a sample of 150mg is advisable. We suggest using a jeweler’s scale to ascertain the weight of the sample. If head hair isn’t available, a comparable volume of body hair can be used. By head hair, we mean scalp hair only. Body hair encompasses all other types of hair (like facial or underarm).

Process Overview

The laboratory processes a drug test result through four major stages: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample undergoes initial integration into the lab’s framework. This step involves checking if the sample is properly sealed and dispatched, generating a unique LAN (Laboratory Accessioning Number), and inputting any extra data not covered by an electronic custody chain.

Screening entails a preliminary overview to detect substance abuse. While economically efficient for disproportionately negating drug usage in most samples, a positive screen result warrants corroboration for courtroom validity. Samples showing preliminary positive results in Screening necessitate secondary confirmation.

Should a sample exhibit a potential positive during Screening, an extra portion of the original hair sample is taken for Extraction. Here, drugs are extracted from hair at a significantly lower concentration compared to other methods (e.g., urine or saliva), making hair drug analysis a complex procedure.

Confirmation of any positive initial result is handled via techniques like GC/MS, GC/MS/MS, or LC/MS/MS. Samples with suspected positive results undergo cleaning before confirmation as necessary. The whole lab workflow from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair category and complies with ISO / IEC 17025 benchmarks.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
