For EtG, additional analyses, and/or tests exceeding 10 panels, a suggested 150mg of the sample is preferable. We suggest measuring the sample using a jeweler’s scale. Should scalp hair be insufficient, a comparable amount of body hair can be gathered. Head hair specifically pertains to scalp hair only, while body hair includes all other variations (facial, axillary, etc.).

Process Overview

The laboratory processing of drug test results encompasses four essential stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the preliminary integration of a sample into a laboratory’s framework. This process involves verifying that the sample was appropriately sealed and transported, assigning a unique LAN (Laboratory Accessioning Number), and performing any necessary data entry absent from an electronic chain of custody protocol.

Screening is a rapid initial evaluation for drugs of abuse. Although it offers a cost-efficient means to exclude drug use for the majority of samples, a positive result requires confirmation for it to be valid in legal settings. All samples yielding a preliminary positive in Screening necessitate subsequent confirmation.

In instances where Screening indicates a preliminary positive, additional hair is extracted from the original sample for the Extraction phase. During Extraction, drugs are isolated from hair in significantly lower concentrations compared to alternative methods (e.g., urine or oral fluid), underscoring the complexity of hair drug screening.

Confirmation of any positive screen outcome is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All preliminary positive samples are cleansed as needed prior to confirmation. The comprehensive laboratory procedure from Accessioning through Confirmation is conducted under both the CAP (College of American Pathologists) Hair designation and accredited to ISO/IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
