When collected near the scalp, hair can reveal up to about a 3-month detection window for substances. Collection is straightforward, challenging to tamper with, and convenient to transport.

A 1.5-inch section of roughly 200 hair strands (around the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, the optimal sample size for testing and verification. For EtG, additional screenings, and/or assessments exceeding 10 panels, 150mg of the specimen is advised. Using a jeweler's scale for weighing the sample is recommended. If scalp hair isn't available, a similar amount of body hair can be used. When mentioning head hair, we specifically mean scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The drug test laboratory procedure comprises four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the preliminary processing of a sample into a lab's system, which involves verifying proper sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and finishing any extra data entry omitted by an electronic chain of custody system.

Screening offers an initial rapid assessment for substance abuse. Although Screening is an economical method to rule out drug use on most samples, any positives require confirmation to be court-admissible. All samples showing presumptive positives in Screening necessitate secondary validation.

If Screening indicates a presumptive positive, additional hair is extracted from the original sample and prepared for Extraction. At this stage, drugs are extracted at lower concentrations than other methods (e.g., urine or oral fluid), making hair drug testing the most complex method to execute.

Confirmation of any positive screening outcome is executed using GC/MS, GC/MS/MS, or LC/MS/MS. Necessary washing of all presumptive positive samples is performed before confirmation. From Accessioning to Confirmation, the entire lab procedure is reviewed under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
