For EtG, supplemental, and/or tests exceeding 10 panels, 150mg of the sample is advised. Utilizing a jeweler's scale to weigh the sample is suggested. Should scalp hair be unavailable, a similar amount of body hair can be gathered instead. In terms of head hair, only scalp hair is considered. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The primary steps in the laboratory processing of a drug test are Accessioning, Screening, Extraction, and Confirmation.

Accessioning concerns the initial integration of a sample into a lab's system. This step includes ensuring the sample was correctly sealed and shipped, generating a random LAN (Laboratory Accessioning Number), and completing any extra data entry that is absent from an electronic chain of custody framework.

Screening constitutes a preliminary assessment for drug misuse. While Screening is an economical way to dismiss drug usage in most samples, a positive result necessitates confirmation to be valid in a legal context. Samples that are presumptively positive in Screening require an additional confirmation process.

If a sample tests presumptively positive in the Screening phase, more hair is withdrawn from the initial sample and readied for Extraction. In this phase, drugs are extracted from hair at considerably lower concentrations than in alternative methods (ex. urine or oral fluid), which explains why hair drug testing is the most challenging technique to execute.

Confirmation for any positive screening outcome is performed through GC/MS, GC/MS/MS, or LC/MS/MS. All samples that are presumptive positive are cleansed before confirmation as necessary. The overall laboratory process from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair classification and is also accredited to ISO / IEC 17025 standards.
