A 1.5-inch segment consisting of about 200 hair strands (approximately the diameter of a #2 pencil) from the scalp provides 100mg of hair, the perfect sample size for assessment and validation. For EtG, additional tests, and/or analyses beyond 10 panels, 150mg of the sample is advisable. We suggest using a jeweler’s scale for precise measurement. Should scalp hair not be available, a comparable quantity of body hair can be collected. In terms of head hair, we specifically mean scalp hair. Body hair includes other types like facial or axillary hair.

Process Overview

The laboratory processing steps for a drug test result consist of Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial process of entering a sample into a lab’s system. This process entails ensuring the sample was sealed and shipped as required, assigning a unique LAN (Laboratory Accessioning Number), and completing any supplementary data input absent from an electronic chain of custody system.

Screening consists of a preliminary evaluation for illicit drugs. Though Screening is a cost-effective method to eliminate drug use in most samples, any positive findings must be confirmed to be court-admissible. Samples with presumptive positives from Screening require secondary validation.

If Screening results indicate presumptive positivity, additional hair from the original sample is extracted and prepared for Extraction. During this phase, drugs are extracted from hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug screening a complex procedure.

Confirmation of positive screening results is performed through GC/MS, GC/MS/MS, or LC/MS/MS methods. Presumptive positive samples are cleansed prior to confirmation when necessary. The comprehensive laboratory procedure from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standard accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
