What it is, How it works

Hair serves as a repository for long-term records of drugs and alcohol by capturing biomarkers within the fibers of the growing hair strand. When gathered near the scalp, hair can offer a detection window of up to roughly 3 months for substances. Collecting hair is straightforward, hard to tamper with, and simple to transport.

A 1.5-inch portion of around 200 hair strands (resembling the size of a #2 pencil) nearest to the scalp will result in 100mg of hair, which is the optimal amount for screening and validation. For EtG, additional tests, and tests exceeding 10 panels, a 150mg sample is advisable. It's suggested to weigh the sample using a jeweler’s scale. If scalp hair is unavailable, an equivalent quantity of body hair may be used. When indicating head hair, we exclusively mean scalp hair. Body hair encompasses all other hair variations (facial, axillary, etc.).

Process Overview

The four primary phases involved in the laboratory processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial processing of a sample into the laboratory’s system. This comprises validating that the sample was correctly sealed and dispatched, allotting a random LAN (Laboratory Accessioning Number), and completing any other data entries not supplied by an electronic chain of custody system.

Screening is the preliminary quick assessment for drugs of abuse. Though Screening is an economical method to exclude drug usage on most samples, a positive result needs verification to be acceptable in legal settings. Any samples appearing presumptively positive in Screening necessitate consequential confirmation.

Should a sample appear presumptively positive during the Screening phase, additional hair is taken from the primary specimen and prepared for Extraction. In this phase, drugs are extracted at notably lower concentrations from hair compared to other methods (ex. urine or oral fluid), which renders hair drug screening the most challenging methodology.

Confirmation of any positive screening outcomes is executed through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as required. The entire laboratory process from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

It served as a prominent platform for knowledge exchange and collaboration, facilitating groundbreaking advancements in hair testing and analysis. In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
