If scalp hair isn't available, an equivalent quantity of body hair can be utilized. The term head hair refers strictly to scalp hair, while body hair encompasses all other types (including facial, axillary, etc.).

Process Overview

The laboratory processing of drug test results consists of Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial handling of a specimen into the laboratory's system. This step ensures the sample is properly sealed and shipped, assigns a unique LAN (Laboratory Accessioning Number), and completes any manual data input not covered by an electronic chain of custody.

Screening involves a preliminary check for illicit substances. Although Screening provides a cost-efficient means to exclude drug use in most specimens, a positive result in this phase must be confirmed for legal acceptability. Samples indicating potential positives in Screening require additional confirmation procedures.

Should a Screening result indicate a presumptive positive, further hair from the original sample is extracted for the Extraction phase. In this stage, drugs are isolated from hair at lower concentrations than found in other testing methods (such as urine or oral fluids), establishing hair drug testing as the most complex method to execute.

Confirmation of positive results from Screening is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples suggesting positives are cleansed before verification if necessary. The complete process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
