Procedure Summary

The lab processes a test result through four primary phases: Accessioning, Screening, Extraction, and Confirmation.

The first step, Accessioning, involves entering the sample into the lab system. It comprises ensuring the sample was sealed and dispatched appropriately, assigning a random LAN (Laboratory Accessioning Number), and completing any extra information not included by the digital custody chain system.

Screening provides an initial, rapid assessment for drug presence. Although Screening is a cost-efficient strategy to exclude drug use in most specimens, a positive result must be validated for legal admissibility. Samples indicating a preliminary positive during Screening must undergo a secondary confirmation.

When Screening shows a presumptive positive, extra hair is retrieved from the original sample for the Extraction phase. Here, drugs are extracted from hair at much lower concentrations than other methods (such as urine or oral fluids), making hair drug screening the most challenging methodology.

Confirmation of any positive result from Screening is done using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with a presumptive positive are cleaned before Confirmation if necessary. The joint meeting of the Society of Hair Testing (SoHT) and the Italian Group of Forensic Toxicologists (GTFI) was organized in Verona, June 8–10, 2022, and hosted by the Legal Medicine team of the Verona University Hospital, directed by Franco Tagliaro together with Federica Bortolotti and Rossella Gottardo.

It served as a prominent platform for knowledge exchange and collaboration, facilitating groundbreaking advancements in hair testing and analysis. In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.
