A sample measuring 1.5 inches with about 200 hair strands (comparable to a #2 pencil) nearest to the scalp will yield 100mg of hair, which is ideal for both initial testing and confirmatory analysis. For EtG additions and/or panels exceeding 10, 150mg is advised. Utilizing a jeweler’s scale for measurement is suggested. In the absence of scalp hair, an equivalent portion of body hair can be used. When discussing head hair, we mean scalp hair exclusively. Body hair encompasses all remaining types (facial, axillary, etc.).

Process Overview

The primary stages in laboratory processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial integration of a sample within a lab’s framework. This step involves ensuring the sample’s proper sealing and shipment, assigning a Laboratory Accessioning Number (LAN), and completing any additional data entry absent in an electronic custody chain.

Screening offers a preliminary quick assessment for drugs of concern. While Screening offers a cost-effective solution to exclude drug use for many samples, a positive result must undergo confirmation for legal acceptance. Samples that are presumptively positive during Screening require further confirmation.

Should a sample test positive in the Screening phase, additional hair is extracted from the original sample for the Extraction process. Here, drugs are retrieved from hair at concentrations much lower than other techniques (e.g., urine or oral fluid), contributing to hair drug screening being the most challenging to execute.

Confirmation of any positive Screening result is accomplished through GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives undergo washing before confirmation as necessary. The complete lab procedure from Accessioning through Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair classification and ISO / IEC 17025 accreditation.
