For EtG, additional tests, and/or those exceeding 10 panels, 150mg of the material is advised. We suggest using a jeweler’s scale for measuring the specimen. If scalp hair isn't available, the same quantity of body hair may be utilized. When mentioning head hair, it specifically denotes scalp hair. Body hair encompasses all other hair types (such as facial, axillary, etc.).

Process Overview

The core stages in the lab processing of a drug screening process are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the preliminary handling of a sample into a lab’s framework. This encompasses confirming that the sample was sealed and delivered correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any additional details not included by an electronic chain of custody system.

Screening serves as a preliminary swift evaluation for substances of abuse. While Screening is an economical method for excluding drug usage in most samples, a positive screening must be verified to be legally valid. Any samples with a presumptive positive screening require a further confirmation.

If a sample is flagged as presumptive positive during the Screening phase, more hair is extracted from the initial sample and readied for Extraction. In this phase, drug compounds are isolated from the hair at a substantially lower concentration compared to other techniques (such as urine or oral fluid), making hair drug testing one of the most intricate methodologies.

Validation of any positive screening results is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples that are presumptively positive are cleansed before confirmation as required. The full lab protocol from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair classification and the accreditation in compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
