Hair testing is recognized as an effective method for identifying drug and alcohol consumption. Hair offers an extended overview of alcohol and other drug usage by capturing biomarkers in the fibers of the developing hair strand. When taken near the scalp, hair can present up to a roughly 3-month detection period for alcohol and other substances. It is easy to collect, somewhat resistant to tampering, and simple to send.
A 1.5-inch portion of about 200 hair strands (roughly the thickness of a #2 pencil) nearest to the scalp will yield 100mg of hair, the optimal amount for analysis and verification. For EtG, additional tests, and/or tests exceeding 10 panels, 150mg of the sample is suggested. We advise using a jeweler’s scale to weigh the specimen. If scalp hair is not available, a corresponding quantity of body hair might be collected. By head hair, we specifically mean scalp hair. Body hair includes all other hair categories (facial, axillary, etc.).
Process Overview
The four primary phases in the lab analysis of a drug test include Accessioning, Screening, Extraction, and Confirmation.
Accessioning entails the initial processing of a sample into a lab’s database. This encompasses confirming the sample was sealed and dispatched correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any remaining data entry not covered by an electronic chain of custody system.
Screening is a preliminary swift assessment for drugs of abuse. Although Screening offers an economical way to exclude drug usage from most samples, a positive screening requires confirmation to be valid in legal settings. Any samples that are presumptively positive in Screening must undergo secondary confirmation.
If a sample tests presumptively positive during Screening, additional hair is taken from the initial sample and readied for Extraction. Here, drugs are removed from hair at considerably lower concentrations than in other methods (such as urine or oral fluid), making hair drug screening the most complex approach to execute.
Confirmation of any positive screening outcome is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleaned before confirmation as required. The comprehensive laboratory process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair specification and the accreditation to ISO / IEC 17025 standards.

Note: Although often called "hair follicle tests", the analysis actually examines the hair strand rather than the follicle itself under the scalp.
