A 1.5-inch segment consisting of roughly 200 hair strands (comparable to a #2 pencil's diameter) adjacent to the scalp yields 100mg of hair, which is the preferred quantity for analysis and confirmation. For EtG, add-ons, and/or examinations beyond 10 panels, it is advised to use 150mg of the sample. Utilizing a jeweler’s scale for measuring the sample is suggested. In the absence of scalp hair, an equivalent quantity of body hair may be obtained. The term head hair specifically refers to scalp hair only. Body hair encompasses all other categories of hair (facial, axillary, etc.).

Process Overview

The primary procedures involved in the laboratory examination of a drug test result encompass Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial entry of a sample into a laboratory’s database. This step includes confirming that the sample was sealed and transported correctly, assigning a unique LAN (Laboratory Accessioning Number), and completing any supplementary data entry not captured by an electronic custody tracking system.

Screening encompasses an initial rapid assessment for drugs of abuse. Screening is a cost-efficient method to dismiss drug consumption for the majority of samples, though a positive result must be substantiated for court admissibility. Samples testing potentially positive at the Screening phase require further confirmation.

Should a sample indicate potential positivity during Screening, an additional hair segment is extracted from the original sample and prepared for Extraction. During this phase, drugs are isolated from hair at notably lower concentrations than alternative methods (such as urine or oral fluid), which is why hair drug assessment presents higher complexity.

Confirmation of any screening result that tests positive is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with potential positivity undergo a washing process before confirmation when necessary. The complete laboratory methodology from Accessioning to Confirmation adheres to both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
