For EtG, additional tests, and those exceeding 10 panels, 150mg of hair is suggested. Utilizing a jeweler’s scale for measurement is ideal. If scalp hair is not an option, an equivalent volume of body hair is acceptable. Reference to head hair pertains strictly to scalp hair, while body hair encompasses all other varieties (facial, axillary, etc.).

Process Overview

The core steps in processing a drug test in a lab include Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample is initially processed into the lab’s system. It involves checking that the sample was correctly sealed and delivered, assigning it a LAN (Laboratory Accessioning Number), and inputting any supplementary data not covered by a digital chain of custody system.

Screening serves as an initial, swift assessment for substances of abuse. Although Screening is an economical method to dismiss drug presence in most samples, a positive result needs further verification to be legally acceptable. Samples indicating potential positivity in Screening must undergo additional confirmation.

When a sample shows potential positivity in the Screening phase, more hair is extracted from the original sample for the Extraction phase. Here, drugs are isolated from hair at significantly lower concentrations than other methods (e.g., urine or saliva), rendering hair drug screening the most intricate methodology performed.

Any positive Screening outcomes undergo Confirmation using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All initially positive samples are cleaned before confirmation if necessary. The complete laboratory protocol from Accessioning to Confirmation is inspected under the CAP (College of American Pathologists) Hair criteria and accredited to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
