What it is, How it works

Recognized as an effective means for discovering drug and alcohol consumption, hair testing stands out. Hair offers a prolonged timeline of alcohol and drug exposure by embedding biomarkers within the strands of the growing hair. When sampled from near the scalp, hair can present a detection period of up to approximately 90 days for alcohol and drugs. Hair is straightforward to procure, relatively challenging to tamper with, and simple to transport.

A 1.5-inch segment encompassing roughly 200 strands of hair (approximately the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, the ideal quantity for screening and confirmation processes. For EtG, additional, and/or analyses surpassing ten panels, 150mg of the sample is advised. It's suggested to weigh the sample on a jeweler’s scale. If scalp hair isn't accessible, an equivalent amount of body hair may be collected. When discussing head hair, we mean scalp hair specifically. Body hair encompasses all other hair categories (facial, axillary, etc.).

Process Overview

The comprehensive steps associated with the lab processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a specimen into a laboratory's framework. This involves validating that the sample was appropriately sealed and dispatched, assigning a unique LAN (Laboratory Accessioning Number), and finalizing any supplementary data input absent from an electronic chain of custody arrangement.

Screening constitutes an initial rapid evaluation for drugs of abuse. Although Screening is a cost-efficient strategy to negate drug usage in most samples, a positive screen necessitates confirmation to be legally accepted. Any samples that screen as presumptively positive warrant a subsequent confirmation.

If a specimen is presumptively positive during Screening, further hair is extracted from the initial sample and readied for Extraction. In this phase, drugs are isolated from hair at a considerably lower concentration than other methodologies (e.g., urine or oral fluid), which underscores the complexity of hair drug screening.

Confirmation of any positive screening result occurs using GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples undergo washing as necessary before confirmation. The comprehensive laboratory procedure from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 criteria.

Advantages of hair drug testing:

  • Sustained detection period: Hair drug tests identify drug consumption up to 90 days, unlike urine tests, which have a briefer detection period.
  • Difficult to subvert: Altering results in a hair drug test is very challenging, enhancing accuracy.
  • Delivers historical data: It reveals patterns of drug use over time, rather than just recent usage.

Limitations:

  • Inability to detect recent intake: Detecting drugs in hair requires approximately 5-7 days.
  • Expense: Generally, hair drug tests cost more than other drug testing approaches.
  • Variable outcomes: Elements like hair color and individual hair growth differences can modify the concentration of drug metabolites in hair.

Note: Even though these are often termed as "hair follicle tests", the analysis is performed on the strand of hair, not the follicle beneath the scalp.
