For EtG, additional, and/or analyses surpassing ten panels, 150mg of the sample is advised. It's suggested to weigh the sample on a jeweler’s scale. If scalp hair isn't accessible, an equivalent amount of body hair may be collected. When discussing head hair, we mean scalp hair specifically. Body hair encompasses all other hair categories (facial, axillary, etc.).

Process Overview

The comprehensive steps associated with the lab processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a specimen into a laboratory's framework. This involves validating that the sample was appropriately sealed and dispatched, assigning a unique LAN (Laboratory Accessioning Number), and finalizing any supplementary data input absent from an electronic chain of custody arrangement.

Screening constitutes an initial rapid evaluation for drugs of abuse. Although Screening is a cost-efficient strategy to negate drug usage in most samples, a positive screen necessitates confirmation to be legally accepted. Any samples that screen as presumptively positive warrant a subsequent confirmation.

If a specimen is presumptively positive during Screening, further hair is extracted from the initial sample and readied for Extraction. In this phase, drugs are isolated from hair at a considerably lower concentration than other methodologies (e.g., urine or oral fluid), which underscores the complexity of hair drug screening.

Confirmation of any positive screening result occurs using GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples undergo washing as necessary before confirmation. The comprehensive laboratory procedure from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 criteria.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
