What it is, How it works

Hair analysis has emerged as an effective method for detecting drug and alcohol consumption. Hair retains a lengthy history of alcohol and drugs by capturing biomarkers within the fibers of the developing hair strand. When harvested near the scalp, hair can offer up to nearly a 3-month window for drug and alcohol detection. Hair is straightforward to obtain, somewhat challenging to tamper with, and simple to send.

A 1.5-inch segment with around 200 hair strands (comparable to a #2 pencil) nearest to the scalp yields 100mg of hair, which is the ideal amount for screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the sample is advisable. It's recommended to measure the specimen on a jeweler’s scale. If scalp hair isn't available, an equivalent quantity of body hair can be collected. When mentioning head hair, we specifically mean scalp hair. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The four principal steps in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial processing of a sample within a lab's system. This involves confirming that the sample was properly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and carrying out any additional data entry not provided by an electronic chain of custody system.

Screening consists of an initial rapid check for substances of abuse. Although Screening is a cost-efficient method to exclude drug use in most samples, a positive screen must be confirmed to be permissible in court. Any samples that are presumptively positive in Screening require subsequent confirmation.

If a sample is presumptively positive in the Screening phase, additional hair is extracted from the initial specimen and set for Extraction. During this phase, drugs are extracted from hair at a significantly lower concentration than other methods (ex. urine or oral fluid), making hair drug screening the most complex methodology to execute.

Confirmation of any positive screening result is executed via GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples are cleansed prior to confirmation as necessary. The comprehensive laboratory process from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
