A 1.5-inch segment consisting of about 200 hair strands (roughly the size of a #2 pencil) closest to the scalp will yield around 100mg of hair, which is the recommended quantity for effective screening and confirmation. For EtG, additional tests, and assessments beyond 10 panels, a collection of 150mg is suggested. It's advisable to weigh the sample with a jeweler’s scale. Should scalp hair not be available, an equivalent amount of body hair can be utilized. When mentioning head hair, we only refer to scalp hair, while body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

Laboratory processing for drug test results includes four essential phases: Accessioning, Screening, Extraction, and Confirmation.

The accessioning phase revolves around the initial processing where a sample is integrated into a lab’s system. This involves verifying proper sealing and shipping of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing additional data entry not covered by an electronic chain of custody system.

Screening provides a quick check for substance abuse. Though cost-effective for ruling out drug use in many cases, a screening test that turns positive requires a confirmation to be valid in court. Any sample showing presumptive positivity in Screening needs secondary confirmation.

Should the Screening find a sample presumptively positive, extra hair from the initial collection is prepared for Extraction. This phase involves extracting drugs from hair at much lower concentrations compared to other methods (e.g., urine or oral fluid), which makes hair drug screening notably challenging.

Confirmation of any positive screening outcome employs GC/MS, GC/MS/MS, or LC/MS/MS techniques. Necessary washes are performed on all presumptive positive samples before confirmation. The whole laboratory procedure from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair standards and the adherence to ISO / IEC 17025 accreditation.
