What it is, How it works

Hair testing is recognized as an effective method for identifying alcohol and drug consumption. By capturing biomarkers in the fibers of a growing strand, hair offers long-lasting evidence of substance use. If hair is sampled close to the scalp, it can reveal up to a three-month window for detecting drugs or alcohol. The collection is straightforward, hard to tamper with, and effortless to transport.

A 1.5-inch segment of about 200 hair strands (roughly equivalent to a #2 pencil) near the scalp will provide 100mg of hair, which is ideal for initial screening and validation. For EtG, additional tests, or panels exceeding ten, a sample weighing 150mg is advised. We suggest using a jeweler’s scale to ascertain the specimen's weight. If scalp hair cannot be accessed, an equal amount of body hair is acceptable. When we mention head hair, we refer exclusively to scalp hair. Body hair encompasses all other types (like facial or axillary hair).

Process Overview

The laboratory process for deriving a drug test result includes four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial incorporation of a sample into the laboratory's framework. This step involves confirming the sample was properly sealed and dispatched, assigning a unique LAN (Laboratory Accessioning Number), and completing any additional data entry absent from an electronic chain of custody system.

Screening offers a preliminary rapid assessment for potential drug use. Although Screening is an economical means of eliminating drug usage in most cases, a positive result must be verified to be legally valid. Any presumptively positive samples from Screening require further confirmation.

If a sample indicates a presumptive positive during Screening, further hair is extracted from the initial sample for Extraction. At this stage, drugs are isolated from hair at a much lower concentration than in other techniques (like urine or oral fluid), making hair drug screening among the most challenging methodologies.

Confirmation of any positive screening outcome is conducted using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples with presumptive positive results undergo washing before confirmation, as necessary. The entire lab process, from Accessioning to Confirmation, is evaluated according to the CAP (College of American Pathologists) Hair designation and in compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
