A sample measuring 1.5 inches and consisting of around 200 hair strands (comparable to the width of a #2 pencil) nearest the scalp yields 100mg of hair, deemed the ideal quantity for testing and verification. For EtG, additional panels, and tests beyond 10 panels, a 150mg specimen is ideal. We advise using a jeweler's scale for weighing the sample. Should scalp hair be unavailable, a similar amount of body hair may be gathered. References to head hair exclusively pertain to scalp hair. Body hair encompasses all other hair forms (facial, axillary, etc.).

Process Overview

Four key phases constitute the laboratory handling of a drug test: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial sample processing into a laboratory's system, verifying proper sealing and shipment, assigning a random LAN (Laboratory Accessioning Number), and performing any data entry not covered by an electronic chain of custody system.

Screening involves a preliminary rapid assessment for drug abuse. While Screening is a cost-effective method to exclude drug use in most samples, positive results require confirmation for legal admissibility. Any Screening-positive samples necessitate subsequent confirmation.

If presumptive positivity occurs at the Screening stage, additional hair is extracted from the initial sample and prepared for Extraction. Here, drugs are withdrawn from hair at concentrations significantly lower than in other methodologies (e.g., urine or oral fluid), which complicates hair drug testing.

Confirmation of Screening-positive results is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing before confirmation as necessary. The comprehensive lab process, from Accessioning to Confirmation, is scrutinized under the CAP (College of American Pathologists) Hair designation and the ISO/IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
