A 1.5-inch segment of about 200 hair strands (around the size of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the ideal amount for testing and confirmation. When testing for EtG, add-ons, or over 10 panels, 150mg of hair is suggested. It’s advisable to weigh the hair on a jeweler’s scale. If scalp hair isn't available, a similar quantity of body hair can be utilized. "Head hair" refers to scalp hair exclusively. "Body hair" encompasses all other hair types (e.g., facial, axillary).

Process Overview

The key stages in the lab processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial process of integrating a sample into a lab's system. This covers checking that the sample was sealed and sent correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry that an electronic chain of custody system does not provide.

Screening is a preliminary quick test for drugs of abuse. Though Screening is a cost-efficient way to rule out drug usage in most samples, a positive result requires confirmation to be valid in legal proceedings. Samples that are presumptively positive in Screening necessitate a secondary confirmation.

If a sample is potentially positive during Screening, more hair is extracted from the original sample for Extraction. During this stage, drugs are extracted from hair at a much lower concentration than in other methods (e.g., urine or oral fluid), which makes hair drug screening the most challenging method to execute.

Confirmation for any positive screening outcome is carried out through GC/MS, GC/MS/MS or LC/MS/MS. All potentially positive samples are cleaned before confirmation as needed. The full lab process from Accessioning to Confirmation is audited under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
