A sample measuring 1.5 inches with roughly 200 hair strands (approximately the thickness of a #2 pencil) taken nearest the scalp provides 100mg of hair, which is ideal for analysis and verification. For EtG, additional evaluations, and tests exceeding 10 panels, it is suggested to use 150mg of the sample. We recommend measuring the hair using a jeweler’s scale. If scalp hair is not available, an equivalent amount of body hair may be used. When mentioning head hair, we specifically mean scalp hair. Body hair includes all other varieties (like facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four primary stages: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is initially recorded into a laboratory’s system. This process entails verifying that the sample was sealed and shipped correctly, allocating a random LAN (Laboratory Accessioning Number), and finishing any extra data input not supplied by an electronic chain of custody system.

Screening involves a preliminary rapid examination for substance misuse. While it is a cost-efficient method to exclude drug use in most samples, a confirmatory test is necessary to make a positive screening result admissible in legal settings. Consequently, samples flagged as positive in the Screening phase necessitate additional confirmation.

Should the Screening phase classify a sample as presumptively positive, additional hair is extracted from the original collection and readied for Extraction. Here, substances are isolated from hair at a lower concentration compared to other methods (such as urine or saliva), making hair drug screening the most challenging to execute.

Confirmation for any positive screening outcome is executed through GC/MS, GC/MS/MS, or LC/MS/MS. All samples that are presumptively positive are cleansed prior to confirmation, as needed. The entire laboratory process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
