In cases involving EtG, additional tests, and/or tests exceeding 10 panels, a 150mg sample is advised. We advocate for using a jeweler's scale to weigh the specimen. If scalp hair isn't available, an equivalent amount of body hair may be collected. When mentioning head hair, we specifically mean scalp hair. Body hair encompasses all other hair types (e.g., facial, axillary).

Process Overview

The laboratory processing of a drug test result involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into the laboratory's system. This step involves confirming the sample's proper sealing and shipment, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry not captured by an electronic chain of custody system.

Screening is a preliminary rapid assessment for drug abuse. Although Screening is a cost-efficient method to rule out drug use for most samples, a positive result demands confirmation for court admissibility. Samples returning a presumptive positive in Screening require a confirmatory secondary test.

If a sample returns a presumptive positive at the Screening phase, additional hair is retrieved from the initial sample for preparation in the Extraction phase. During Extraction, drugs are retrieved from hair in significantly lower concentrations compared to other methods (e.g., urine, oral fluid), which positions hair drug testing as the most complex strategy to execute.

Confirmation of any positive Screening result is executed through GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive undergo necessary washing before confirmation. The full laboratory procedure from Accessioning to Confirmation undergoes evaluation under the CAP (College of American Pathologists) Hair designation and is aligned with ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
