What it is, How it works

Seen as a potent method for identifying drug and alcohol consumption, hair testing allows for a prolonged detection history by capturing biomarkers within the growing hair strand fibers. When collected close to the scalp, hair can provide up to a roughly three-month detection period for alcohol and other drugs. Hair is easy to collect, less susceptible to tampering, and simple to send.

A 1.5-inch sample of about 200 hair strands (approximately the size of a #2 pencil) near the scalp will yield 100mg of hair, the perfect sample for screening and confirmation. For EtG, add-ons, and/or tests over 10 panels, 150mg of the specimen is advised. We suggest weighing the specimen on a jeweler’s scale. If scalp hair isn’t available, an equivalent amount of body hair may be gathered. When we mention head hair, we're referring to scalp hair only. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processes a drug test result through four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails initially processing a sample into a lab’s system. This involves confirming that the sample was sealed and sent properly, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not supplied by an electronic chain of custody system.

Screening consists of an initial rapid check for drugs of abuse. While Screening offers a cost-effective means to rule out drug usage in the majority of cases, any positive screens need verification to be legally acceptable. Any samples that are presumptively positive in Screening necessitate secondary confirmation.

Should a sample test as presumptively positive during the Screening stage, more hair is drawn from the initial sample and prepped for Extraction. During this phase, drugs are extracted from hair at a significantly lower concentration than in other methods (e.g., urine or oral fluid), which is why hair drug screening is the most challenging method to perform.

Confirmation of any positive screening result is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed before confirmation as required. The entire lab process from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and accredited to ISO / IEC 17025 standards.

Advantages of hair drug testing:

  • Extended detection window: Hair drug tests can reveal drug use for up to 90 days, whereas urine tests have a shorter detection timeframe.
  • Hard to manipulate: Cheating a hair drug test is very challenging, ensuring more precise results.
  • Offers historical insight: It provides a pattern of drug use over time, beyond recent use.

Limitations:

  • Cannot detect immediate use: Drugs become detectable in hair approximately 5-7 days after use.
  • Expense: Hair drug tests are typically pricier than other testing methods.
  • Variation in results: Factors like hair color and individual differences in hair growth can influence drug metabolite concentration in the hair.

Note: Although termed "hair follicle tests," the analysis actually examines the hair strand, not the follicle beneath the scalp.
