For EtG, additional tests, and/or analysis beyond 10 panels, collecting 150mg of hair is advised. We suggest weighing the collected sample on a jeweler’s scale. If scalp hair is not obtainable, an equivalent quantity of body hair may be used. When mentioning head hair, we refer exclusively to scalp hair. Body hair encompasses all other types (e.g., facial, axillary).

Process Overview

Accessioning pertains to the initial integration of a sample into a lab's system. This step verifies the sample’s proper sealing and shipping, assigns a random LAN (Laboratory Accessioning Number), and finalizes any necessary data entry omitted by an electronic chain of custody system.

Screening comprises a preliminary swift assessment for substances. Although Screening efficiently rules out drugs in most samples cost-effectively, a positive screen requires substantiation to stand in legal proceedings. Presumptive positives in Screening necessitate further confirmation.

Upon obtaining a presumptive positive at the Screening phase, additional hair is extracted from the main sample and set for Extraction. Drugs are extracted from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), pinpointing hair drug screening as the most intricate procedure to execute.

Confirming any positive screening outcome is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples undergo washing ahead of confirmation as required. The comprehensive laboratory process from Accessioning to Confirmation undergoes scrutiny under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
