What it is, How it works

A 1.5-inch segment with approximately 200 hair strands (around the dimension of a #2 pencil) nearest to the scalp supplies 100mg of hair, which is the optimal quantity for testing and verifying. For EtG, add-ons, and/or evaluations exceeding 10 panels, 150mg of the sample is advised. It is suggested to measure the sample on a jeweler’s scale. If head hair is not present, a comparable amount of body hair may be utilized. When mentioning head hair, it pertains solely to scalp hair. Body hair refers to all other types (facial, axillary, etc.).

Process Overview

There are four primary layers in the laboratory handling of a drug screening outcome: Accessioning, Screening, Extraction, and Confirmation.

Accessioning comprises the initial processing of a specimen within a lab's system. This process includes validating that the sample was sealed and dispatched correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any necessary data registration not offered by an electronic chain of custody framework.

Screening entails an initial rapid assessment for substance misuse. While Screening is a budget-friendly approach to ruling out substance consumption on most samples, a positive finding must be validated to be acceptable in legal contexts. Any samples that indicate a presumptive positive in Screening require additional validation.

If a sample suggests a presumptive positive at the Screening phase, more hair is extracted from the initial sample and prepared for Extraction. In this phase, substances are isolated from hair at a much lower concentration than other methods (for instance, urine or oral fluid), highlighting why hair substance analysis is the most complex method to execute.

Validation of any positive screening outcome is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All samples indicating presumptive positivity are rinsed before validation as required. The comprehensive lab procedure from Accessioning to Validation is audited under both the CAP (College of American Pathologists) Hair categorization and the accreditation in compliance with ISO / IEC 17025 guidelines.

In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
