Process Overview

The laboratory processing of drug test results comprises four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the preliminary handling of a sample into a lab’s system. This step verifies the sample's proper sealing and shipping, allocates a random LAN (Laboratory Accessioning Number), and finalizes any data entry not facilitated by an electronic chain of custody system.

Screening is a preliminary evaluation for drugs of abuse. While it is a cost-efficient method to negate drug use in most samples, a positive screen must be confirmed for legal admissibility. Samples that are likely positive in Screening require a secondary confirmation.

If a sample shows presumptive positivity during Screening, additional hair is extracted from the initial specimen and prepared for Extraction. This step entails extracting drugs from hair at much lower concentrations than other methods (e.g., urine or oral fluid), making hair testing a more complex process.

GC/MS, GC/MS/MS, or LC/MS/MS is used for confirming any positive Screening result. All samples displayed as presumptive positive are cleaned before confirmation as needed. The full lab process, from Accessioning to Confirmation, is audited under the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
