In the absence of scalp hair, a similar quantity of body hair can be gathered. References to head hair imply scalp hair exclusively, while body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four significant steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial processing of a sample within a laboratory’s system. This step involves verifying that the sample was sealed and dispatched correctly, assigning a random LAN (Laboratory Accessioning Number), and incorporating any additional data entry not facilitated by an electronic chain of custody system.

Screening includes a preliminary assessment for drugs of misuse. Although Screening efficiently excludes drug usage for the major part of samples, a positive test requires verification for legal acceptance. Each presumptive positive sample in Screening necessitates subsequent confirmation.

When a sample shows a presumptive positive result in the Screening stage, more hair is extracted from the initial sample and readied for Extraction. This phase involves drug extraction from hair at significantly lower concentrations than in other processes (e.g., urine or saliva), making hair drug testing one of the most complex methodologies to execute.

Confirmation of any positive screen is undertaken through GC/MS, GC/MS/MS or LC/MS/MS. Every presumptive positive sample is cleansed before confirmation as necessary. The comprehensive laboratory procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
