A 1.5-inch segment comprising around 200 hair strands (comparable to the thickness of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the prime sample size for both screening and confirmation. For EtG, additional tests, and/or analyses exceeding 10 panels, 150mg of the specimen is advised. It is advisable to weigh the specimen using a jeweler's scale. If scalp hair is unavailable, a similar quantity of body hair can be considered. When mentioning head hair, it pertains solely to scalp hair. In contrast, body hair includes all other hair kinds (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result primarily includes the steps of Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial sample processing into a laboratory's system, ensuring the sample is sealed and dispatched correctly, assigning a LAN (Laboratory Accessioning Number) randomly, and finalizing any extra data entry not supplied by an electronic chain of custody system.

Screening constitutes an initial swift assessment for substance misuse. Although Screening offers a cost-effective solution for excluding drug use across most samples, a positive screen necessitates confirmation to be court-admissible. Samples showing presumptive positive results in Screening require an additional confirmation.

Should a sample indicate a presumptive positive during the Screening phase, further hair is extracted from the primary specimen for preparation in the Extraction phase. Here, drugs are retrieved from hair at a notably lower concentration than via other techniques (e.g., urine or oral fluid), making hair drug screening the most daunting technique to execute.

Confirmation of any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptively positive samples undergo washing prior to confirmation as necessary. The complete laboratory process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standard accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
