A 1.5 inches segment of roughly 200 hair strands (similar to the girth of a #2 pencil) nearest to the scalp will yield 100mg of hair, an ideal quantity for testing purposes. For EtG, additional tests, and/or panels exceeding 10, a 150mg specimen is suggested. It is advisable to weigh the sample using a jeweler's scale. If scalp hair isn't an option, equivalent body hair might be useful. When we mention head hair, it strictly refers to scalp hair. Body hair encompasses other types (facial, underarm, etc.).

Process Overview

The primary phases involved in lab processing of a drug test result comprise Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial intake of a sample into a lab’s system, which includes ensuring that the sample was securely sealed and shipped, assigning a unique LAN (Laboratory Accessioning Number), and fulfilling any additional information entry not covered by an electronic chain of custody system.

At this point, drugs are drawn from hair at significantly lower concentrations than through other methods (for instance, urine or saliva), making hair drug screening the most intricate approach to conduct.

Verification of any positive Screening outcome is executed through GC/MS, GC/MS/MS, or LC/MS/MS. All samples showing presumptive positives are cleansed prior to confirmation as required. The full lab procedure from Accessioning to Confirmation is conducted under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
