What it is, How it works

Hair testing is now recognized as a formidable method for identifying the use of alcohol and drugs. With hair taken near the scalp, it can show up to a three-month detection period for substances. Hair collection is straightforward, not easily tampered with, and convenient to transport.

A sample of hair measuring 1.5 inches, consisting of approximately 200 strands (around the diameter of a #2 pencil) nearest to the scalp will yield 100mg, which is ideal for both screening and verification. For EtG, extra tests, and tests exceeding 10 panels, 150mg of hair is suggested. It is advisable to weigh the sample with a jeweler’s scale. If scalp hair is not an option, an equivalent amount of body hair can be used. When mentioning head hair, it specifically refers to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

Laboratory processing of a drug test result consists of four essential steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves starting the sample process into a lab’s system. It includes ensuring the sample is properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data required beyond what an electronic chain of custody system provides.

Screening is a preliminary check for the presence of drugs. Though it is cost-efficient for eliminating drug use in most samples, a positive result must be confirmed to be valid in a legal context. Any samples found presumptively positive during Screening must undergo secondary confirmation.

If Screening indicates a presumptive positive, more hair is taken from the original sample for Extraction. This stage extracts drugs from hair at a significantly lower concentration compared to other methods (such as urine or oral fluid), which is why hair screening is the toughest to perform.

Confirmation of any positive found in Screening is done using GC/MS, GC/MS/MS, or LC/MS/MS. All initially positive samples undergo a wash before confirmation as needed. The entire process from Accessioning to Confirmation is examined under the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
