What it is, How it works

Hair testing is now viewed as a potent method for identifying drug and alcohol consumption. Hair captures biomarkers within the strands, offering a prolonged history of alcohol and drugs. When collected near the scalp, it can reveal a detection timeframe of up to roughly 3 months. Hair collection is straightforward, difficult to tamper with, and easy to transport.

A sample of 1.5 inches, consisting of approximately 200 hair strands (roughly the width of a #2 pencil) nearest the scalp yields 100mg of hair, ideal for both screenings and confirmation. For EtG, add-ons, and/or tests beyond 10 panels, 150mg of hair is suggested. Using a jeweler’s scale for weighing the sample is recommended. If scalp hair is inaccessible, a similar amount of body hair may be used. "Head hair" refers specifically to scalp hair, while "body hair" denotes all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the first processing step, integrating a sample into a lab’s system. This step ensures the sample was properly sealed and shipped, assigns a LAN (Laboratory Accessioning Number), and completes any extra data entry not covered by an electronic chain of custody system.

Screening is a preliminary rapid assessment for drugs. While cost-effective for ruling out drug use in most samples, a positive screen must be corroborated to be legally valid. Samples presumed positive in Screening need confirmation with a secondary step.

For samples presumed positive in Screening, more hair is extracted from the initial sample for the Extraction process. Here, drugs are extracted from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), making hair drug screening the most complex method to execute.

Confirmation of any positive Screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive undergo a wash before confirmation, if necessary. The entire laboratory process from Accessioning through Confirmation is subject to review under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.
