Process Overview

The laboratory analysis of a drug test result encompasses four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning handles the initial integration of a specimen into a laboratory’s system. It encompasses the verification of the sample’s proper sealing and shipment, allocation of a random LAN (Laboratory Accessioning Number), and completion of any outstanding data entry not addressed by an electronic chain of custody system.

Screening allows for an initial rapid assessment for drugs of abuse. Though cost-efficient for excluding drug use in most samples, a positive screen demands confirmation for court acceptability. Presumptively positive samples identified during Screening necessitate subsequent confirmation.

During the Extraction stage, if a sample tests presumptively positive in Screening, additional hair is retrieved from the original sample for processing. Drugs are extracted from hair at a much lower concentration compared to other methods (such as urine or oral fluid), highlighting the complexity of hair drug screening.

Confirmation of any positive screen outcome occurs via GC/MS, GC/MS/MS, or LC/MS/MS. As needed, washing of all presumptive positive samples is performed before confirmation. From Accessioning to Confirmation, the comprehensive lab process is evaluated under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
