For EtG, add-ons, or testing above ten panels, a specimen weight of 150mg is suggested. It's advisable to use a jeweler’s scale to weigh the specimen. If scalp hair isn't available, an equivalent quantity of body hair can be gathered. In this context, head hair refers specifically to scalp hair, while body hair encompasses all other hair types (such as facial and axillary).

Process Overview

The laboratory procedures for processing a drug test result consist of four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial sample entry into the laboratory's system. This step includes ensuring the sample was sealed and shipped correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry absent from an electronic chain of custody system.

Screening provides a preliminary rapid assessment for drug abuse. Though cost-effective for ruling out drug use in most samples, a positive screen must be confirmed for court acceptance. Samples labeled presumptively positive in Screening necessitate additional confirmation.

In cases of presumptive positivity in Screening, more hair is extracted from the primary specimen for the Extraction stage. This stage involves extracting drugs from hair at markedly lower concentrations than other methods (e.g., urine or oral fluid), highlighting the complexity of conducting hair drug screening.

GC/MS, GC/MS/MS, or LC/MS/MS techniques are used to confirm any positive screening results. Presumptive positive samples undergo washing before confirmation when required. The entire laboratory process from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair classification and accreditation to ISO/IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
