For EtG, additional panels, and tests above 10 panels, a 150mg sample is advised. It's recommended to weigh the sample with a jeweler’s scale. If scalp hair is not available, an equal quantity of body hair may be used. "Head hair" specifically refers to scalp hair only, while "body hair" includes all other types (facial, axillary, etc.).

Process Overview

The process of laboratory testing for drug results involves Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the preliminary processing of a sample into the laboratory’s system. This step verifies that the sample is properly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and completes any auxiliary data entry absent from an electronic chain of custody system.

Screening serves as an initial rapid assessment for drugs of abuse. While it is an economical means of identifying drug usage in most samples, a positive screening result must be confirmed for legal validity. Samples that are potentially positive in Screening necessitate a secondary confirmation.

If a sample tests potentially positive during Screening, additional hair is extracted from the original specimen and prepped for Extraction. In this phase, drugs are extracted from hair at a concentration lower than in other methods (e.g., urine or oral fluid), making hair drug screening the most complex method to conduct.

Confirmation of any positive screening outcome is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo a wash before confirmation if required. The complete laboratory process from Accessioning to Confirmation is audited under both the CAP (College of American Pathologists) Hair specification and the ISO/IEC 17025 accreditation standards.
