What it is, How it works

When obtained close to the scalp, hair can offer nearly a three-month window of detection for alcohol and drugs. Hair collection is straightforward, comparatively difficult to tamper with, and simple to transport.

A sample measuring 1.5 inches consisting of roughly 200 hair strands (approximately the width of a #2 pencil) close to the scalp yields 100mg of hair, which is ideal for analysis and confirmation. For EtG, additional tests, and/or panels exceeding 10, a specimen weighing 150mg is advised. It's advisable to weigh the specimen using a jeweler’s scale. If scalp hair isn't available, an equivalent amount of body hair can be used. Reference to head hair implies scalp hair exclusively. Body hair denotes any other hair type (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test consists of four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial input of a sample into the laboratory’s system. This stage includes ensuring the sample is sealed and delivered correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any missing data entry not covered by an electronic chain of custody process.

Screening involves a preliminary assessment for substances of abuse. Although it is a cost-effective approach for initially ruling out drug usage on most samples, a positive result needs confirmation for legal acceptability. Any samples that test positive on Screening require a follow-up confirmation.

If a sample tests positive during Screening, additional hair is extracted from the original specimen for Extraction. At this point, drugs are extracted from hair at much lower concentrations compared to other methods (ex. urine or oral fluid), making hair drug screening the most complex method to conduct.

Confirmation of any positive screening outcome is achieved using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples undergo washing before confirmation as necessary. The entire laboratory workflow from Accessioning to Confirmation is evaluated according to both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 criteria.

Advantages of hair drug testing:

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
