A sample of 1.5 inches, containing about 200 hair strands (roughly the diameter of a #2 pencil) closest to the scalp, provides 100mg, the perfect amount for analysis. For tests such as EtG or those exceeding 10 panels, 150mg of hair is preferred. We advise using a jeweler’s scale to measure this. If head hair cannot be collected, an equivalent amount of body hair is suitable. "Head hair" denotes scalp hair exclusively, while "body hair" refers to all other types (like facial and axillary hair).

Process Overview

There are four integral steps in the lab processing of drug test samples: Accessioning, Screening, Extraction, and Confirmation.

Accessioning concerns the preliminary handling of a sample by the laboratory, ensuring it is sealed and shipped correctly, assigning a LAN (Laboratory Accessioning Number), and completing any necessary data entry absent from an electronic custody chain.

Screening provides an initial assessment for substances of concern. Though cost-effective, it requires confirmation of any positive results to ensure legality. Samples that test presumptively positive at this stage necessitate further validation.

Should a sample show presumptive positivity in Screening, more hair is extracted from the initial sample for further analysis. Drugs are isolated from hair in lower concentrations than other methods (e.g., urine or saliva), making hair testing complex.

Confirmation of any positive results proceeds using GC/MS, GC/MS/MS, or LC/MS/MS technologies. Each presumptive positive sample is pre-washed as necessary. The entire testing process from Accessioning to Confirmation undergoes scrutiny under CAP (College of American Pathologists) Hair guidelines and is compliant with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
