A specimen of 1.5 inches consisting of about 200 hairs (similar in size to a #2 pencil) nearest to the scalp yields 100mg of hair, which is optimal for testing. For tests surpassing 10 panels, EtG, or add-ons, a specimen of 150mg is suggested. It is advisable to weigh the specimen using a jeweler's scale. If scalp hair is inaccessible, an equivalent quantity of body hair may be used. By scalp hair, we imply the hair on the head. Body hair encompasses other types of hair (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result comprises four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the introductory process of integrating a sample into the lab's system. This step includes ensuring the sample was properly sealed and dispatched, allocating a random LAN (Laboratory Accessioning Number), and completing any missing data entry not available from an electronic chain of custody system.

Screening offers a preliminary rapid assessment for substances of abuse. This method efficiently excludes drug use in most samples; however, a confirming procedure is required for a screen result to be court-admissible. Consequently, samples that presumptively test positive during Screening necessitate further confirmation.

When Screening indicates a presumptive positive, additional hair is extracted from the initial sample and processed for Extraction. In this phase, drugs are recovered from the hair at significantly lower concentrations compared to other methods (such as urine or oral fluid), accounting for the complexity of conducting hair drug screening.

Confirmed positive results from screenings are conducted using GC/MS, GC/MS/MS or LC/MS/MS techniques. Prior to confirmation, all samples that appear presumptively positive are washed as needed. The complete lab procedure from Accessioning to Confirmation is audited under both the CAP (College of American Pathologists) Hair designation and aligned with ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
