Hair tests are favored by companies with strict no-drug policies, judicial bodies, and those on probation due to their extended detection period, typically up to 90 days, and their resistance to manipulation. However, for drug use detection within the last 5 days, urine tests remain more accurate.

What it is, How it works

Hair testing is recognized as an effective method for detecting drug and alcohol consumption. It allows for a prolonged monitoring period of substances, as the growing hair strands trap biomarkers within their fibers. When collected near the scalp, the hair can offer a detection span of up to three months for both alcohol and drugs. The process of collecting hair is straightforward, with lower chances of tampering, and it is also convenient to transport.

Collecting a 1.5-inch segment of around 200 strands of hair (comparable to the girth of a #2 pencil) from the scalp yields about 100mg, which is the optimal amount needed for testing and verification. For EtG, additional tests, or panels exceeding ten, 150mg of the sample is advised. We suggest using a jeweler’s scale to measure the specimen. If scalp hair is unavailable, an equivalent quantity of body hair can be taken. Note that when mentioning head hair, we specifically mean scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

There are four primary steps in processing a drug test result in the lab: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the first step, where the sample is logged into the lab's system. This involves confirming that the sample was properly sealed and delivered, allocating a random LAN (Laboratory Accessioning Number), and completing any data entry not already covered by an electronic chain of custody system.

Screening offers a preliminary quick assessment for drug presence. Although Screening efficiently rules out drug use in most samples at a lower cost, any positive screens must undergo Confirmation to hold legal validity. Samples testing presumptively positive in Screening require secondary confirmation.

If a sample shows positive indications at the Screening phase, additional hair is removed from the original sample for Extraction. During Extraction, drugs are retrieved from the hair at much lower concentrations compared to other methods (such as urine or oral fluid), making hair drug testing notably challenging.

Confirmation of any positive Screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples undergo washing before Confirmation as required. The whole lab process from Accessioning to Confirmation is assessed in accordance with both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
