For EtG, additional testing, and tests exceeding 10 panels, 150mg is advisable. Using a jeweler’s scale to measure the specimen is suggested. If scalp hair is not available, the same quantity of body hair can be used. When discussing head hair, it pertains to scalp hair only. Body hair includes other types (facial, axillary, etc.).

Process Overview

The four essential steps in processing a drug test result at the lab are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves entering the sample into the lab’s system. It checks that the sample was sealed and sent correctly, assigns a random LAN (Laboratory Accessioning Number), and completes any needed data entry absent in an electronic chain of custody system.

Screening checks quickly for drugs of abuse. While efficient for ruling out drug use in most cases, a positive screen needs confirmation to be court-admissible. Samples that are presumptively positive in Screening must undergo secondary confirmation.

If Screening indicates a presumptive positive, more hair is taken from the initial sample for Extraction. In this step, drugs are extracted from hair in significantly lower concentrations than other methodologies (e.g., urine or oral fluid), making hair drug screening the most challenging method to conduct.

Confirmation of any positive screening results is done via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are washed before confirmation if needed. The whole laboratory process from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
