What it is, How it works

A 1.5-inch piece consisting of around 200 hair strands (approximately the thickness of a #2 pencil) closest to the scalp yields 100mg, which is the optimal sample size for both screening and confirmation. For EtG, additional tests, or panels exceeding 10, a 150mg sample is advisable. It is suggested to weigh the sample on a jeweler’s scale. If scalp hair is unavailable, a similar quantity of body hair can be used. When we mention head hair, we specifically mean scalp hair. Body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The primary stages involved in the lab processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves introducing a sample into the lab's system, which includes validating that it was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and performing any required data entry not covered by an electronic chain of custody system.

Screening entails an initial rapid check for drugs of abuse. While Screening is a cost-efficient approach to exclude drug use in most samples, a positive result must be confirmed to be legally valid. Presumptive positive results in Screening necessitate a secondary confirmation.

If the sample tests positive during the Screening phase, additional hair is extracted from the original sample for the Extraction process. In Extraction, drugs are isolated from hair at a significantly lower concentration than in other methods (such as urine or oral fluid), which makes hair drug testing a challenging procedure.

GC/MS, GC/MS/MS, or LC/MS/MS is used to validate any positive result from Screening. Presumptive positive samples are cleansed before confirmation when necessary. The entire lab procedure from Accessioning to Confirmation is scrutinized under the CAP (College of American Pathologists) Hair designation and follows the accreditation to ISO / IEC 17025 standards.
