For EtG, additional tests, or tests above 10 panels, 150mg of the sample is advised. We suggest using a jeweler’s scale to weigh the sample. If scalp hair is unavailable, an equivalent quantity of body hair may be obtained. Regarding head hair, we specifically mean scalp hair. Body hair encompasses all other hair varieties (such as facial or axillary hair).

Process Overview

Laboratory processing of a drug test result involves four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves initially processing a sample into the lab's system. This step includes confirming proper sealing and shipping of the sample, assigning a random Laboratory Accessioning Number (LAN), and finishing any extra data entry not supplied by an electronic chain of custody system.

Screening is a preliminary inspection for substance abuse. Although Screening is a cost-effective method to eliminate drug use for most samples, a positive screen must be confirmed to be valid in legal settings. Samples that are presumptively positive in Screening require further confirmation.

If a sample tests presumptively positive during Screening, additional hair is taken from the original sample for Extraction. During Extraction, drugs are derived from hair at a much lower concentration than with other methodologies (such as urine or oral fluids), highlighting why hair drug screening is one of the most complex methodologies.

Confirmed positive screening results use GC/MS, GC/MS/MS, or LC/MS/MS. Any presumptively positive samples undergo washing before confirmation, as needed. The entire lab process, from Accessioning to Confirmation, is evaluated under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
