Over the past years, drug testing methods increasingly favor hair follicle drug tests. Employers, courts, and substance abuse experts often prefer these over traditional urine tests. Employers adhering to zero-tolerance drug policies, courts, and probationary individuals rely on hair follicle tests. Hair analysis is recognized as an effective means for identifying drug and alcohol consumption. The structure of hair allows it to capture biomarkers over time within the fibers of its growing strands. When obtained near the scalp, hair offers a detection window of up to about three months for alcohol and other substances. Collecting hair is straightforward, hard to tamper with, and convenient for shipping.
A section measuring 1.5 inches from roughly 200 hair strands (approximately the diameter of a #2 pencil) near the scalp yields 100mg of hair, which is the optimal amount for testing and validation. For EtG, additional examinations, and/or analyses exceeding 10-panel tests, 150mg of the sample is advisable. We suggest using a jeweler’s scale for weighing the sample. If scalp hair is unavailable, an equivalent quantity of body hair can be used. When we mention head hair, we mean scalp hair exclusively. Body hair encompasses all other hair types (facial, axillary, etc.).
Process Overview
The lab processing of a drug test result involves four major steps: Accessioning, Screening, Extraction, and Confirmation.
Accessioning is the first phase, where a sample is entered into a lab’s system. This step includes ensuring that the sample was sealed and transported correctly, assigning a random LAN (Laboratory Accessioning Number), and finishing any extra data entry not handled by an electronic chain of custody system.
Screening is an initial rapid assessment for substance abuse. Although Screening is cost-efficient for excluding drug use in most samples, any positive result requires confirmation to be valid in legal contexts. Samples presumptively positive in Screening require additional confirmation.
Should a sample show a presumptive positive during Screening, more hair is taken from the initial collection for the Extraction phase. Here, drugs are extracted at a much lower concentration compared to other methods (e.g., urine or oral fluid), which makes hair drug analysis the most complex to execute.
Confirmation of any positive Screening results is carried out via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples are washed before confirmation as necessitated. The entire lab procedure from Accessioning to Confirmation is scrutinized according to the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

Note: Though commonly called "hair follicle tests", the assessment targets the hair strand and not the follicle beneath the scalp.
