For EtG, additional testing, and/or tests exceeding 10 panels, 150mg of the sample is advised. We advise weighing the specimen on a jeweler’s scale. If scalp hair is absent, an equivalent quantity of body hair might be taken. When discussing head hair, we refer only to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of drug test results involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary processing of a sample into the lab's system. This step includes confirming the sample's proper sealing and shipment, allocating a random LAB (Laboratory Accessioning Number), and finalizing any additional data entry absent from an electronic chain of custody system.

Screening offers an initial, rapid examination for substances of abuse. This stage involves extracting drugs from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), which makes hair drug screening the most challenging methodology to execute.

Confirmation of any positive screening outcome is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All samples showing presumptive positive results are cleansed as needed before confirmation. The full lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
