A sample length of 1.5 inches, consisting of roughly 200 strands near the scalp (comparable to the thickness of a #2 pencil), provides 100mg of hair, which is the preferred amount for testing and confirming results. For EtG, additional tests, or panels exceeding 10, a 150mg sample is advisable. Using a jeweler’s scale is suggested for weighing the sample. If scalp hair cannot be obtained, a similar amount of body hair might be used. When discussing head hair, we mean scalp hair only. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The primary steps in processing a drug test result through a lab include Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial stage of processing a sample into a lab's system. It involves verifying that the sample was correctly sealed and sent, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry that an electronic chain of custody might not provide.

Screening entails a preliminary check for drug presence. Screen testing is an economical way to discount drug usage in most samples, yet a positive screen warrants confirmation for legal acceptance. Samples that test positive during Screening need further confirmation.

When a sample indicates positive in the Screening phase, more hair is extracted from the initial sample for the Extraction phase. During this phase, drugs are removed from hair at a significantly lower concentration than other methods (such as urine or oral fluid), highlighting the complex nature of hair drug screening.

Confirmation of a positive screening result utilizes GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positives undergo pre-confirmation washing as necessary. The comprehensive lab process from Accessioning to Confirmation is evaluated under both CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standard accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
