Hair captures a prolonged history of substance use by embedding biomarkers within the fibers of the growing hair. When collected near the scalp, it offers up to a 3-month timeframe for detecting alcohol and other drugs. Hair is easy to gather, somewhat challenging to tamper with, and simple to transport.

A sample measuring 1.5 inches with approximately 200 hair strands (about the diameter of a #2 pencil) closest to the scalp provides 100mg of hair, the ideal amount for testing and confirmation. For EtG, additional tests, and/or panels exceeding 10, a 150mg sample is suggested. We recommend using a jeweler’s scale to weigh the specimen. If scalp hair isn't available, a comparable amount of body hair may be used. When discussing head hair, we refer exclusively to scalp hair. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The primary steps in laboratory testing for drug results are Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the first step, processing a sample into the lab’s system. This involves ensuring the sample was correctly sealed and shipped, assigning a LAN (Laboratory Accessioning Number) randomly, and entering any supplementary data not provided by an electronic chain of custody.

Screening provides an initial evaluation for drugs of abuse. Although Screening is a cost-efficient approach to potentially rule out drug use in most samples, any positive finding needs validation to be court-admissible. Any samples with presumptive positivity during Screening must undergo a secondary confirmation.

Should a sample test presumptive positive in Screening, additional hair is retrieved from the original specimen for Extraction. Drugs are extracted from hair at a significantly lower concentration than via other methods (e.g., urine or oral fluid), which makes hair drug testing the most intricate methodology to carry out.

Confirmation for any positive screening outcome is executed through GC/MS, GC/MS/MS, or LC/MS/MS techniques. Presumptive positive samples undergo a washing process before confirmation if necessary. The complete laboratory sequence from Accessioning to Confirmation is scrutinized according to CAP (College of American Pathologists) requirements and ISO / IEC 17025 accreditation standards.
