For EtG, additional tests, and those exceeding 10 panels, 150mg of the sample is advised. It is suggested to weigh the sample on a jeweler's scale. If scalp hair is not available, a comparable quantity of body hair may be used. Mention of head hair strictly pertains to scalp hair. Body hair includes other types such as facial and axillary hair.

Process Overview

The processing of a drug test result in the lab consists of four principal steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning comprises the preliminary processing of a sample into a lab's system. This process entails confirming that the sample was sealed and delivered properly, assigning it a random LAN (Laboratory Accessioning Number), and inputting any supplementary data not provided by an electronic chain of custody system.

Screening conducts a preliminary rapid check for drug use. While Screening is a cost-effective method to eliminate drug use for most samples, a positive result in Screening requires confirmation for court acceptance. All samples that test positive in Screening must be verified through secondary confirmation.

Upon a positive result in the Screening phase, more hair is harvested from the initial sample and readied for Extraction. During Extraction, drugs are removed from hair at markedly lower concentrations than other methods (e.g., urine or oral fluid), which is why hair drug testing is considered the most complex method.

Confirmation of any positive Screening result is executed through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation as necessary. The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
