Hair is easy to gather, relatively hard to alter, and convenient to transport.

A 1.5-inch segment of around 200 hair strands (roughly equivalent to a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the optimal sample size for both initial and confirmatory testing. For EtG, additional analyses, and/or tests exceeding 10 panels, 150mg of the specimen is advisable. We advise using a jeweler’s scale to measure the specimen. Should scalp hair be lacking, an equivalent volume of body hair can be obtained. When we mention head hair, we strictly mean scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The four primary phases in the lab handling of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the preliminary entry of a sample into a lab’s framework. This step ensures the sample was properly sealed and transported, assigns a random LAN (Laboratory Accessioning Number), and completes any further data input not supplied by an electronic custody chain system.

Screening constitutes an initial rapid test for substances of concern. While Screening is a cost-efficient method to eliminate drug presence in most specimens, a positive screen needs confirmation for eligibility in legal proceedings. Samples that test presumptively positive during Screening necessitate a secondary validation.

If a sample is presumptively positive during Screening, additional hair is collected from the original specimen for Extraction. Within this stage, drugs are retrieved from hair at significantly lower concentrations compared to other methods (such as urine or oral fluid), making hair drug tests exceptionally challenging to execute.

Confirmation of any positive screening result is achieved via GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples designated as presumptive positives undergo washing before confirmation as required. The full laboratory procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.
