When sourced close to the scalp, hair can present nearly a 3-month period for identifying alcohol and drug presence. Collection of hair is straightforward, relatively challenging to tamper with, and conveniently shipped.

A 1.5-inch portion of roughly 200 hair strands (equivalent to the circumference of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the optimal sample for both screening and verification. For EtG, extensions, and/or tests exceeding 10 panels, 150mg of the sample is suggested. It is advisable to measure the sample weight using a jeweler’s scale. If scalp hair cannot be obtained, a comparable quantity of body hair may be gathered. References to head hair pertain specifically to scalp hair. Conversely, body hair includes all other types (facial, axillary, etc.).

Process Overview

The four primary stages in the laboratory analysis of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary handling of a sample within a laboratory's database. This step involves ensuring the sample is correctly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and finalizing any supplementary data input not accounted for by an electronic chain of custody system.

Samples exhibiting presumptive positivity in Screening need secondary confirmation.

Should a sample be presumptively positive at the Screening phase, additional hair is extracted from the original specimen and arranged for Extraction. During this phase, drugs are extracted from hair at much lower concentrations compared to other techniques (e.g., urine or oral fluid), which accounts for the complexity of hair drug screening.

Confirmation of any positive Screening result occurs through GC/MS, GC/MS/MS, or LC/MS/MS methods. All samples with presumptive positivity undergo cleansing before confirmation if needed. The entirety of the laboratory process, spanning from Accessioning to Confirmation, is conducted under the scrutiny of both the CAP (College of American Pathologists) Hair designation and the accreditation conforming to ISO / IEC 17025 benchmarks.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
